When you register with the Practice you will be asked to fill in a medical card or complete a registration form and questionnaire requesting details of your medical history and any medication you are taking A convenient appointment will be arranged for you to see a healthcare assistant to check your history and do a basic health check, if appropriate due to COVID-19.

You do not need to register unless you physically need to see a GP and have been booked an appointment at our Practice.
You can print off the relevant forms (below), or open and save onto your PC and email to the Practice. Please complete BOTH the GMS1 form and the New Patient Questionnaire. If you do not submit a GMS1 form we cannot legally register you.
The GMS1 form is a PDF document. You will need Adobe Reader to open this and edit, you can download this for free from the Adobe Website.

Registration forms can be emailed to Registrations.W92039@wales.nhs.uk.
Once you have been registered, you can apply for a My Health Online account.
If you wish to request a copy of your medical records, blood results, hospital letters etc., please complete a Subject Access Request Form and send it to us via eConsult. Once received, we can send you requested copies via eConsult. Full copies of medical records due to their size cannot be emailed but will be made available to you.
PERMISSION TO SHARE A MEDICAL RECORD
If you would like someone to speak to to the practice on your behalf, please complete a Patient Consent Form and return it to the surgery. You will be contacted by telephone to verify your identity and we will update your record accordingly.
