Privacy Notice

Please read our Privacy Notice for Patients and Carers which gives guidance on what to expect when you, and sometimes others, provide your personal information to us.  It sets out what information we collect about you and why we collect it, how the information may be used, who it may be shared with and how we will protect it and keep it confidential. 

It explains what rights you have to control how we use your information, our legal basis for processing it and how you can access it. We also explain who to contact if you have any questions and how to contact them.

IMPORTANT - information about the General Practice Data for Planning and Research 
This is a new data service developed by NHS Digital to collect data from GP medical records to support health and care planning and research in England. 

This data service does not presently apply to General Practices' in Wales and is specifically for those Practices providing contracted services to English residents within England.  

There is no requirement for patients registered in Wales to inform their GP of "Dissent from secondary use of GP patient identifiable data' either via a template, letter, email or verbally as the processing of information for this purpose does not currently apply to Welsh GP held data.
