The Practice does not supply letters of support for requests from patients requesting health information, proof of diagnosis, condition affecting life, environmental etc for any organisations, for example: 

  • Housing
  • Benefits - PIP, universal credit, ESA 
  • Blue badge
  • Grants/ energy grants 
  • Travel 
  • Employment capability / occupational health 
  • Education - exams, attendance etc 

Letters are considered private work and the GP Partners are not required contractually to undertake non-NHS work.  

A computer printout of diagnosis, medication, hospital clinic letters, consultations can be provided free of charge to patients to assist with completion of forms. 

Companies requesting reports should write to the GP Practice directly.
